Change of Address This online system allows your to easily notify the University of a change to either your permanent or term-time address. Use your LSBU user name and password to login and your changes will be updated on our student record overnight.	Link
Change of Course Form This form is for use by students who wish to change their course. Students who wish to change their course will need to discuss this with academic staff and obtain their written permission on the form.	PDF 373KB

Letter of Confirmation Request Form If you completed your studies more than a year ago, this form can be used to request a letter of confirmation. This will state your attendance and award. A charge of £10 is made for this service payable by cheque or postal order.	PDF 337KB

Replacement of University Award Certificate Form This form is for use by students who need to request a replacement award certificate. Until the summer of 1992 the body which awarded degrees at London South Bank University was the Council for National Academic Awards (CNAA).
